Satisfaction surveys, applications, feedback, event sign-ups
LiveForms & Surveys
Build a form from a template, reach customers with a link or QR code; answers are collected together with the privacy notice and the results show up in a report.
Forms & Surveys lets a business ask its customers questions and collect the answers in an orderly way. You start from a ready-made template or a blank form and build the questions yourself. When you publish, you get a link and a QR code. The customer fills the form in on their phone without creating an account. Every form carries a privacy notice and, if you like, a separate consent box; results appear in the panel as distributions, averages and NPS. The Tech Service customer satisfaction survey uses the same core.
Your account opens with Forms & Surveys. It stays open for 15 days, no credit card needed.
How it works
- 01
Pick a template, build the questions
Start from the satisfaction, NPS, feedback, application or event template; add and reorder questions, set conditional questions.
- 02
Write the privacy notice and settings
Set the privacy notice, consent box, anonymous mode, closing date and retention period; publish.
- 03
Share the link or QR code
Create a public link and QR code, or a personal single-use link. You can send it over WhatsApp to one person from your own phone.
- 04
See the results
Distributions, averages and NPS show in the panel; you download the answers as CSV.
Who it’s for
- Any business that wants to measure customer satisfaction
- Organisations that collect applications or requests
- Teams taking event sign-ups and attendance
- Tech service and after-sales teams
Example
A service shop, a satisfaction survey
An illustrative flow; it does not describe a real business.
- Day 1
The manager picks the “Customer satisfaction” template: an overall rating, a recommendation question and a note field. They check the privacy notice, set the low-score threshold and publish.
- Day 1
They download the QR code of the public link and print it for the counter.
- Day 2
A customer scans the code, fills the form in on their phone without an account, reads the privacy notice and ticks the consent box.
- Day 2
One customer gives a low score; managers get a notification in the panel.
- Month end
The manager sees the average rating and NPS on the results screen and downloads the answers as CSV.
Live today
Building forms
- Question types: short and long text, single and multiple choice, rating (1-5), NPS (0-10), date, email, phone, checkbox
- Ready-made templates: customer satisfaction, NPS, feedback and suggestions, application form, event registration
- Required questions, help text and conditional questions that depend on an earlier answer
- Write questions in Turkish, English and Azerbaijani; customers see them in their own language
- Every published version is kept unchanged; editing later creates a new version and old answers stay tied to theirs
- A form can be closed, reopened or archived; it is not deleted
The form on the customer’s phone
- Customers fill it in without logging in, in the phone’s browser; no app needed
- A public link and QR code: used by many people; open until the closing date
- A personal single-use link: answered once, expires, and you can revoke it any time
- Bot protection: request limits and a hidden trap field; the page is closed to search engines
- You cannot view the link address again later; copy it or download the QR code when it is created
Privacy and personal data
- A privacy notice is required on every form; a draft with your company name is pre-filled, and your business and lawyer finalise it
- An optional separate consent box; you can make it required before sending
- The notice the customer saw is kept with the answer, tied to its version
- Raw IP addresses are not stored
- If words for special-category data such as health, religion or ethnicity appear in a form, the panel warns you; publishing needs explicit confirmation
- Retention is set between 30 days and 10 years (default 2 years); when it expires, text, email, phone and date answers are deleted and scores and choices stay anonymously for statistics
Results and anonymous surveys
- Results screen: average and distribution for rating questions, the NPS score, counts and percentages for choice questions
- Answers are listed one by one and download as CSV
- Anonymous mode: answers are not linked to anyone, the one-by-one list and CSV are off, and text answers are shown shuffled
- Small-group threshold (3 to 50, default 5): results are hidden while responses are below the threshold
- The anonymous setting cannot be changed after the first publication
Low scores and Tech Service
- Choose the score question and the low-score threshold: below it, form managers get a notification in the panel
- In Tech Service, a single-use satisfaction link is created for a delivered work order; the answer shows on the work order
- The Tech Service screen summarises the number of answers, the average score, NPS and the number of low scores
Roles and security
- Two roles: Form Manager (builds, publishes, creates links) and Results Viewer (results and answers only)
- In CSV, cells that Excel could run as formulas are escaped
Works together
Because the blocks share one core, these links work today.
- Service & Work Orders
The Tech Service customer satisfaction survey uses the Forms & Surveys core: a link is created for a delivered work order and the answer shows on the work order.
Good to know
- Forms & Surveys sends no email, SMS or bulk messages to customers; the business shares the link and QR code by hand, and can send them over WhatsApp to one person from its own phone.
- There is no file-upload field; customers cannot attach files or photos to a form.
- A form opens as a single page; there are no multi-page forms or calculated scores. Conditional questions work with a single condition.
- Notifications come only on a low score and only in the panel; there is no notification for every new response.
- Responses past their retention period are anonymised when the form’s results or answers are opened and when a new response arrives; there is no scheduled automatic job, and it can also be run by hand from the panel.
- The default privacy notice is a draft; the business is the data controller, and the business and its lawyer finalise the text. This is not legal advice.
The shared core underneath
The same in every block: users, permissions, security and records.
- Your own company space
- Each company’s data is isolated from others and lives at its own address.
- Roles and permissions per app
- A separate role per user per app; managers see only their team.
- Two-factor authentication
- A code sent by email; turned on per company.
- Audit log
- Who changed what, and when — recorded.
- You choose where your data lives
- AWS Frankfurt (EU) by default; on request, your own servers (on-prem).
- Interface in three languages
- English · Turkish · Azerbaijani. Arabic can be added on request.
